My 1998 Durango with a 5.2 only blows hot air when you idle...

My 1998 Durango with a 5.2 only blows hot air when you idle it up, then i t cools right back down when you let off the gas.  I tried back flushing the heater core, but could it be bad?  How do i change the heater core?
