my year old shih-tzu has had four episodes where he was fine one minute and the next has a very tight abdomen, cries in pain when i pick him up, will not get up, i have taken him to our vet three times they ran blood tests, took back x-rays, and have told me there is nothing wrong, within 3-5 days he seems to be back to his normal self. he has no vomiting or diarreha, just very still , hard belly, and cries when you pick him up help...

Oh my. Poor little dog. Perhaps you should take him to another vet?

I have a shih tzu as well the only advice I have for you is to not pick him up by the tummy or any other dog it could hurt their tummy. Also I think you should spend more time with your dog and stop feeding stuff that may cause  vomiting or diarreha

hopes this helps!
