In the game 14.1, is the cue ball spotted behind the head...

In the game 14.1, is the cue ball spotted behind the head spot for the second and subsequent racks, as in the opening break, or is the cue ball left in it's last position?

When one ball is left on the table, the 14 pocketed balls are re-racked, leaving the foot spot open...the cue ball remains where it was and the game continues.

The cue ball stays where it lies.  This is really the essence of the game, breaking out the rack with the last object ball and the cue ball in their last spots.
