What residency program are you in. easiest way to get waiver is by conrad 30 program. start looking and see which states are waiver friendly in your field of residency. Apply for permanent license in pgy2 for that state ,because you will have better position for job approval if you have permanent license. And definitely contact an immigration lawyer.

I am a second year resident in Family Medicine and I am on a J1 visa.  I havent started my J1 waiver yet.  I just contacted our rural health officer assigned for J1 waivers in my state.  He responded to me that he will help me land one.  Does that mean, that I will just wait for him to email me back or what?  Also, do i have to pay the processing fee already so that i can get case number from the dept of state. Please help me out! Thanks!
