Contact a service dog agency in your region.  Domesti-pups has an office in Lincoln, NE and Montgomery, AL.  You can visit our web at www.domesti-pups.org

In regard to the answer about contacting Domesti-PUPS.  My wife is a former volunteer for that group.  She resigned in protest to the training and placement policies and techniques she experienced.  It was a very traumatic experience.  Before becoming affiliated with a service dog group, check them out.
