sjirk,
I have a 2000 tdi with 85000 on it and am having quite the same problem. It will start ok when very cold and the glow plug light stays on for a long time. It will start if engine warm. When it finally does fire up, after several tries, there is a huge billow of smoke.
Did you ever have the Mass Air Flow Sensor go out? That was the downfall on my car.
I had NO power in fact would slow down if going up a hill, cruise control would not maintain. So I had the timing belt changed, the MAFS exchanged and the turbo cleaned out to the tune of $1,200.00. After that the rough idle started, and the hard starting started and kept getting worse. I spent 300 at a shop that thought they could fix it which got me a new battery and cleaning of the throttle assembly. the starting problems persisted and 10,000 miles (6 months) after the MAFS and turbo cleanout, I had a dealer look at it and tell me the turbo needed boiling out. I had the same problems 2 days and $1,100.00 later.
It does not take long to sense I am very bitter. Any savings I had from getting 45 miles to the gallon are long gone and then some.
At this point, unless VW has some balls and does something, I am totally turned off with VW and TDI technology.
The first time they cleaned they cleaned out the turbo, there was a drastic change in power. I had power from 1800 to redline with no lag or loss.

check to see if the agc valve.  ( it is a vacuum controled assembly that is near your left side of the air intake that is hanging up.  If this is sticking, it will cause intermittant starts.  I had mine replaced under warranty and all is well.
