What do you mean by "vagina"? You can get your labia ("lips") pierced, usually the inner ones, or the clithoral hood or even the clitoris itself. The problems for labia piercings are the same as for any piercings - it could get infected if you don't take care of it, or your body could reject it. For the clit hood, the same problems too. For the clitoris itself, the same also applies but you could potentially also lose sensation in the clitoris. For genital piercings definitely see a pro piercer who has lots of experience doing the piercing you are interested in, and talk to them about what you want done.
