See your program sponsor, he or she can issue a new DS2019 form for you. You can stay in the US legally even if your visa has expired. However, if you decide to go out of the states, you will need to reapply for a new J1 visa. You have to understand that there is a difference between a visa and a status(eg. J1, H1 etc). The most important papers are your status, and the DS2019 form. The visa just gets you into the country.

call the INS that is responsible your your own country and for your specific state. For in Kansas (german residents) it's chicago general consulate.
Hope that helped you a little bit.
