The closest one I can come up with is
Godfather Part 2 (1974).

the first sequel was in 1916, "Fall of a Nation," a follow-up to D.W. Griffith's 1915 feature "Birth of a Nation." The first “part 2” sequel however, wasn’t until the Godfather and the first “2” was the French Connection 2.

I am pretty sure it was lion king 2. There were hundreds of movie sequels before that, but that was the first to have the 2 in the second title.
