Why does my Medical Aid in South-Africa refuse to pay for Zaditen as a chronic medication?  What is this international protocol, that they say they subscribe to, which prevents them from putting Zaditen on their formularium for Asthma medication?

This might relate to the prescibed minimum benefits which are applicable to South African medical aids. (see http://www.selfmed.co.za/pmb.asp)
In particular point
3.1 The Beneficiary knowingly declined a formulary drug that is clinically appropriate and effective and opted to use another drug for treatment of a PMB’s condition;
