I am having a hard time house breaking my little girl Nikki....

I am having a hard time house breaking my little girl Nikki.  She is 5 months old and still does not care where she goes even in her crate.  If I leave the back door open then she will opt to go out on her own about 50% of the time.  Nikki usually likes to go to the bathroom rug, or by any ANY door.  I have done the treats and praise, the your a bad girl etc.  Yet it does not seem to be soaking in.  I love her so much and she is SO great except for this one thing.  Also I have tried to train her for a harness and leash - she will not walk when she as those on - she is glued to the ground what can I do to make this an easier adjustment.

You ask a lot of very good questions.  Rather than answering each one here, which will get very long, I am going to recommend that you get the book Chihuahuas for Dummies.  This is the best book I have seen on the care and training of these adorable creatures.
