Sophie, this tag line comes from David Cronenburgs 1986 version of The Fly, check out further info at http://us.imdb.com/title/tt0091064/

For what it's worth, the words "Be afraid, be very afraid" are also used for Mel Smith's 1997 film Bean, but somehow I don't suspect this is the film you were thinking of.

It was also used in one of the Addam's Family movies (Addam's Family Values, I think).

it comes of the 2000 film the Haunted mansion and numerous other horror films

It comes from the old movie, "The Fly" with Jeff Goldblum.  It has been used many times since, usually as parody.
