how do you re-wire an extension cord, what wire goes where? black - ground ... green... white.

Black wire goes to the brass colored screw, white to the white looking screw and green to the green looking screw. Hold the plug facing you with ground down then hot will be on the right and neutral on the left, check both ends to ensure proper installation.
